What Should I Bring to My Initial Appointment?
- Valid State Issued Photo ID (Driver's License or Passport)
- Copy of Insurance Card (Primary and Secondary Insurance)
- Copy of Recent Imaging, Lab Reports, Medical Reports
- List of Current Medications with dosage and preferred pharmacy information
- Copy of Prescription Cards
- New Patient Packet

What Should I Expect on My Initial Consultation?
On your initial consultation, the doctor will conduct a thorough neurological exam, review your past medical records and current medications. This may take up to an hour or more. Based on this information, the doctor may order additional tests, procedures, labs, imaging or physical therapy. He will then enumerate a treatment plan that gets the best possible outcome for your recovery.

What If I Don't Have Insurance?

What if I Need to Cancel/Reschedule My Appointment?
No-Show/Cancellation Policy
Thank you for trusting us with your medical treatment. We strive to provide exceptional neurologic care to all of our patients. In order to serve you better and be fair to other patients who are waiting to seek medical treatment at our office, we have implemented the No-Show/Cancellation Policy effective May 1st, 2023. Please note, you will be charged $25 for each No-Show appointments.
Please cancel/reschedule your appointment with at least 24 hours notice.
So please plan appointments accordingly and notify the office in a timely manner.

Do you Offer Telehealth Services?
As of January 1st, 2025, we will not be offering Telehealth services, except under exceptional cases such as distance and complexity of the patient's disability.

How Do I Access My Health Records?
Please login in to your patient portal to acess your health records

How Do I Get Rx Refills?
Please contact your pharmacy for Rx refills. Allow 3-4 business days for the refills from the providers. Refill requests on weekends will be addressed only on the following business day.

How Do I Access My Labs and Imagings?
All labs and imaging results will be discussed on your FU appointment with the provider. We do not discuss results over the phone. So please refrain form calling the office for lab and imaging results.
